Notice is hereby given that I, Hazel Sylvia Bailey of 151 College Road, London NW10, formerly called known by the name of Hazel Cynthia Bailey, a Citizen of the United Kingdom by virtue of section 11(1) of the British Nationality Act 1981, give public notice that on 10 August 2010 I formally and absolutely renounced and abandoned the use of my name of Hazel Cynthia Bailey and assumed and adopted in place thereof the name of Hazel Sylvia Bailey and further that such change of name is evidenced by a deed poll dated 10 August 2010 under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 21 September 2010.
