Notice is hereby given that a Deed Poll dated Monday 24 January 2011 and enrolled in the Central Office of the Senior Courts on 25 February 2011, as persons having parental responsibility on behalf of the said Rhys James Cook who is a child, single and a British Citizen under section 1(1) of the British Nationality Act 1981 abandoned the name of Rhys James McCallum and assumed the name of Rhys James Cook .
