Notice is hereby given that a Deed Poll dated and enrolled in the Supreme Court of Judicature on 9 May 2011 as person/persons having parental responsibility on behalf of the said Jack Charles Butler-Jones who is a child single and a Commonwealth/British Citizen under section 1(1) of the British Nationality Act 1981 abandoned name of Jack Charles Butler and assumed the name of Jack Charles Butler-Jones.
