Notice is hereby given that a Deed Poll dated and enrolled in the Supreme Court of Judicature on 9 May 2011 as person/persons having parental responsibility on behalf of the said Jessica Ellen Butler-Jones who is a child, single and a Commonwealth/British Citizen under section 1(1) of the British Nationality Act 1981 abandoned name of Jessica Ellen Butler and assumed the name of Jessica Ellen Butler-Jones.
