Notice is hereby given that a Deed Poll dated 8 July 2011 and enrolled in the Supreme Court of Judicature on 28 December 2012, I, Victoria May Scott, as a person having parental responsibility on behalf of the said Cody Adam Jay Campbell (a minor) who is a child single and a British Citizen under section 1(1) of the British Nationality Act 1981 abandoned the name of Cody Adam Jay Scott and assumed the name of Cody Adam Jay Campbell.
