Notice is hereby given that a Deed Poll dated 4 February 2013, and enrolled in the Senior Courts of England and Wales on 27 March 2013, as person having parental responsibility on behalf of Olaf Aleksander Carr, who is a child and single and a British Citizen under section 1 of the British Nationality Act 1981, abandoned the name of Olaf Aleksander Ordak and assumed the name of Olaf Aleksander Carr.
